I think it’s just the one friend and the one rival, with the other being “known”, but not friends.
You have all of your friends. You pick one who is particularly close and one who is a rival.
They way I run it is this: You’re Acquainted with everyone on the list, and more importantly, you can use any one of them to get +1d to a roll (just explain how you’re approaching them about it). However, the Friend is the only one that will do it for free. The rest will ask for something in return. Yes, that includes the Rival/Enemy.
Also, I allow players to start Long Term Projects to turn Acquaintances into Friends.
