I know italics are “free” items, large/heavy items are 2 boxes, but what do the bold items signify?

Bold text signifies a “Fine” item, if memory serves. Basically, better Effect.
Ben is correct, but I’ll be a tiny bit of a pedant ^_~
Quality +1. So if you’re Crew Quality 1, and you have a bold item, it counts as Quality 2 for purposes of effect comparison.
