Start with “What is the goal of the duel?” Kill a man? Shame him? Establish a Rep?
What is the Tier of the NPC gunslinger? Is the PC even in his ballpark? A Tier 0 PC going up against a Tier 2 NPC is gonna be Desperate. And Effect is likely to be Limited.
Once Position and Effect are determined, let the player roll away. But I’d make failure nasty. Like “You’re dead” nasty. Still wanna roll?
Well, I did a Weird West hack (#aFistfulofDarkness) and so the gunslinging came up often. My short answer: It’s just a vanilla Action roll like all other Blades rolls. Like Arne Jamtgaard already said and from the book: Player states the goal and chooses action rating (kill, wound, shame, …), GM sets position + effect level (quality/tier, potency, scale) and go. Again, I’m with Arne concerning wounds (but this depends on how cinematic your tables wants it): If you’re hit, you’re in trouble.
