You can look at the pdf and see the list of fonts in almost any pdf reader program.
The specific font for the title and sub-titles is Kirsty and you can download it for free from Font Squirrel.

The three fonts are listed in the first few pages of the book, I believe on the copyright page. I believe they’re the only three fonts used.
Kirsty and UglyQua (if I’m remembering that name right) are both available online for free. Kirsty is the one for titles, as Paul Baldowski linked.
MinionPro is not available for free, unfortunately.
Yep. Ugly Qua is only used to denote hand written content in the example forms and sheets for the game.
Linux Libertine is a good free alternative to Minion.
